Multiple action items created for 1 question and scoring

Another suggestion for large scale inspections is to have some sort of negative scoring system that is calculated from action items created during inspections. As when there is multiple failures of the 1 type the score is not really reflective of the compliance of the site. As in scaffolding and many other industries their could be multiple failures of the same issue and for accurate analytics there needs to be a score attached to each action item you create so percentage based scoring doesn’t give accurate scores. As I could have a site that has 100 compliance issues of the same type scoring an almost perfect score opposed to a site that has 5 issues in different types causing a lower score than the 1 with 100 issues of the same type. I have combated this by adding questions about multiple failures of any question in this section heavily weighted in scoring to bring some balance but still far from perfect.
