A tool like Microsoft Forms is much better for something like this.  If you really wanted to take advantage of SafetyCulture, I think you could just make your survey in an inspection template and give conduct access to all who need to complete it.  To compile results, you could make an analytics dashboard with charts for each question.

Feel the survey is ok. Ms forms way more advanced. 

The survey could if thought through more than it's current format be very effective.

Again it needs to link from inspections, issues, heads up, training, assets, issues and training.
